Introducing for the first time, the Cedarmak Subscription Medication Plan. This plan was designed to lower out-of-pocket prescription costs to a flat monthly fee for elligible uninsured patients or patients with high deductible plans.

Patients enrolled may recieve up to 5 generic prescriptions for one month supply each per 30 day period from our pharmacy formulary.*

Members with commercial insurance acknowledge that prescriptions filled under this plan are processed as cash transactions and are not submitted to insurance. This subscription medication plan is not insurance and does not replace health insurance coverage. This plan is not available to patients enrolled in or eligible for Medicare, Medicaid, Tricare, or any other government-funded health program.
